UHC Update for State Continuation Coverage Premium Assistance

United Healthcare employer communication and employer verification form for state continuation coverage premium assistance under the American Rescue Plan Act
United Healthcare has released a communication and employer verification form for employers subject to state continuation. This form is to be used for former employees currently on state continuation who qualify for Premium assistance under the American Rescue Plan Act, as well as any employees who become eligible during the period of April 1, 2021, through September 30, 2021.
As previously communicated, The American Rescue Plan Act of 2021 provides temporary premium assistance for employees who had an involuntary loss of coverage or a reduction in hours. The subsidy is available from April 1, 2021, for a maximum of 6 months for any former employees who qualifies. For any newly enrolled state continuation candidates that qualify between the dates of 4/1/2021 and September 30, 2021, the member will receive the subsidy from the effective date of the qualifying event up until September 30, 2021.
