IHC Embedded 2025/2026 Horizon Enrollment Application

Horizon BCBSNJ requires continued use of the Horizon Broker Portal as it automatically ties designated agents to your agency and provides online notifications of approved or disapproved applications typically within 48 hours of our receipt. NJDOBI recently made updates to the attached OFF Exchange application to meet new data requirements for ICHRA and QSEHRA enrollments as part of the application process.
The attached embedded application should be used for special circumstances only, as manual turnaround times can take up to 14 calendar days for review and even longer if there are data matching issues or missing information. These embedded applications will help eliminate commission discrepancies for unregistered NPNs or missing agent/agency information when the portal is not used.

Horizon Non-Group Enrollment/Change Request
Please note:
- The Special Enrollment Period (SEP) feature is currently available on the Horizon Broker Portal.
- Horizon BCBSNJ will be unable to identify agents who are not registered in Miramar Agent.
- Payment for existing consumers is not necessary whether through the portal or if submitted via paper application.
- Dental and Vision Enrollments should also be submitted via the Horizon Broker Portal to ensure accurate and timely processing.
- Please remind agents to utilize the Get Covered New Jersey platform for all ON Exchange Marketplace consumers
