Annual Certifications Reinstated Beginning With October 1, 2022 Anniversary Dates

With the end of New Jersey’s COVID-19 Public Health Emergency declaration, Horizon will reinstate the annual certification process for mid-size groups renewing on or after October 1, 2022.

As a reminder, groups will be required to complete and submit:

• An audit form
• A current Employee Census
• The Large Employer Certification form

Groups will receive the audit link via email prior to the renewal release. We will notify groups of the reinstatement of the certification process.
