HealthEZ is a third-party administrator that combines a customized, self-funded solution with Horizon’s trusted provider networks, to enable employers to:

• Gain greater control and transparency into health care spending and claims performance
• Customize benefit plans tailored to their workforce, without added administrative burden
• Offer a responsive member experience through intuitive tools and service support

We will begin accepting requests for proposals on July 1, 2026, for January 1, 2027 effective dates.
