Horizon to Transition Commission Structure to Per-Contract-Per-Month (PCPM) Model

Applies to: Small Group and Midsize Markets 

To help manage rising healthcare costs, Horizon will transition its commission structure from a percentage-of-premium model to a per-contract-per-month (PCPM) model.

This change will take effect January 1, 2027, and applies to:

  • Small Employer and Midsize Horizon Level Select business
  • Midsize Fully Insured business

Small Employer – Horizon Level Select

  • A flat $70 PCPM will apply to all new and existing Small Group Horizon Level Select accounts
  • Existing groups will convert to the PCPM structure at renewal beginning
    January 1, 2027

Midsize New Business – Horizon Level Select & Fully Insured

  • Commission will be scalable, with:
    • Base: $45 PCPM
    • Maximum: $80 PCPM

Midsize Existing Business

  • Fully Insured Groups
    • 2026 percentage-of-premium commissions will convert to PCPM at renewal beginning January 1, 2027
    • If the converted amount exceeds $80 PCPM, the higher amount will be honored but cannot be increased further
    • Horizon Level Select Groups
      • Commissions will convert to a standard $45 PCPM at renewal beginning January 1, 2027

Commission Change Requests
All commission changes must continue to meet existing transparency and disclosure requirements, including submission of the Broker Commission Change Form.
