Horizon Extends Special Broker Incentive Program

Applies to: Small Employer (2 to 50) and Midsize (51 to 499), excludes Public Sector, National and Labor markets

This special Horizon broker incentive program is extended for all new medical and dental sales with effective dates through December 2025. This special bonus rewards brokers that sell new Fully Insured or new Horizon Level Select groups.

This program includes an additional incentive payment per enrolled member as follows:

Small Group 2 to 50*
$40 per enrolled Member for new business (Horizon Level Select Only)

Midsize Group 51 to 499*
$40 per enrolled Member for new business (Fully Insured and Horizon Level Select)
$10 per enrolled Member for the addition of Horizon Dental coverage** 

 *$20K cap per group
** Medical and Dental must have same effective date
** Applies to Contributory or Voluntary dental coverage
