Horizon Announces Incentive Program for New Level Select Sales 

Horizon is pleased to announce that we are offering a special broker incentive program for all new Horizon Level Select sales with effective dates in June and July 2026.

This program includes an additional incentive payment per enrolled member as follows:

Small & Midsize Groups with five to 250 eligible employees1:

  • $30 per enrolled member for new Horizon Level Select business

For Midsize Groups only1:

  • $10 per enrolled member when dental coverage is added to a Midsize Horizon Level Select New Sale2. 
  • To qualify, medical and dental must have the same effective date and be sold together.
  • This applies to both contributory and voluntary dental coverage.
