Important Changes to Horizon Coverage Options for Small Groups

Horizon continues to look for ways to deliver our products and services more economically and effectively. Therefore, Horizon has elected to withdraw its DA and POS plans from small employer groups headquartered in Ocean County.

What This Means for Your Clients
This withdrawal will affect your group clients headquartered in Ocean County and their employees enrolled in the Horizon Direct Access HSA Gold, Horizon Direct Access Silver, and Horizon Direct Access Platinum plans, as well as Horizon POS Base Plan 100/60 (Platinum).

Affected small group members that have one of these plans can remain in their coverage until their first group anniversary on or after January 1, 2026, at which time these plans will be terminated. These Horizon Direct Access and POS plans will not be available for sale in the Small Group market for groups headquartered in Ocean County as of January 1, 2026.

Selecting a New Health Plan
Please work with your clients well in advance of the termination dates of their plans to explore other health plan offerings to replace the Horizon Direct Access or POS plans they currently offer. Additionally, please work with your group clients to ensure that their employees currently enrolled in these Horizon Direct Access or POS plans to make a new selection.

What to Expect
Within 90 days, and then again within 60 days of their anniversary dates, Horizon will notify your impacted small employer clients about the withdrawal of these health plans.

Our 60-day communication will contain the product(s) we are suggesting replacing the withdrawn plan. We are encouraging your clients to contact you well before their anniversary date to talk about their health and prescription drug coverage options. This communication will be released shortly.
