We are providing advance notice of an upcoming portfolio change that may affect some of your Midsize employer groups.
Beginning with renewals effective March 1, 2027, Horizon will discontinue several legacy Midsize Direct Access, PPO and POS product designs in the M, P, O and Z series. These changes are part of Horizon’s ongoing efforts to maintain a competitive and well-rounded portfolio that better meets the needs of employer groups and members. As a result, these products will no longer be available upon renewal.
Other offerings will be available for consideration, including our new Q-Series portfolio, which features competitive pricing, innovative care navigation, and integrated telemedicine services, as well as enhanced Level Funded options that provide greater choice through updated plan designs and new product offerings.
What You Need to Know

Affected groups will not be automatically moved to another product and will need to determine next steps before renewal.

Horizon representatives will work closely with you to support renewal discussions and evaluate available alternatives.

Groups and members will receive formal notifications approximately 90 days before their renewal date.
